Certified Community Behavioral Health Clinic (CCBHC)

WHAT IS A CCBHC?

Certified Community Behavioral Health Clinics (CCBHCs) are a new provider type in Medicaid, designed to provide a comprehensive range of mental health and substance use disorder services to vulnerable individuals.

CCBHCs are responsible for directly providing (or contracting with partner organizations to provide) nine types of services, with an emphasis on the provision of 24-hour crisis care, utilization of evidence-based practices, care coordination and integration with physical health care.

CLOSING THE LOOP

A CCBHC "closes the loop" across the spectrum of health services, including physical and behavioral health and other social services via CARE COORDINATION.

The CCBHC 9 Types of Services

1. Crisis mental health and addiction services.

2. Screening, assessment and diagnosis, including risk assessment.

3. Person and Family-centered treatment planning.

4. Direct provision of outpatient MH and SUD services.

5. Outpatient primary care screening and monitoring of key health indicators and health risk.

6. Targeted case management.

7. Psychiatric rehabilitation services.

8. Peer support and counselor services and family supports.

9. Intensive, community-based mental health care for members of the armed forces and veterans, particularly those in rural areas.
