Individuals are given information at Intake that include information about Patient Rights.
Advance Directive information is included in the Client’s Rights Procedure.
A formal Psychiatric Advanced Directive is a legal document written by a currently competent person who lives with a mental illness.
Psychiatric Advanced Directive allows a person to be prepared if a mental health crisis prevents them from being able to make decisions.
If an individual notifies you of the document, obtain a copy of the document and put in the medical record.
An Advance Directive allows an individual to specify his/her health care decisions in the event of a life-threatening condition or terminal illness.
An Advance Directive allows an individual to appoint someone as his/her agent to make all health care decisions for them if they become terminally ill and are unable to communicate or make decisions for themselves
• Knock before entering rooms (offices, bedrooms etc.)
• Introduce yourself
• Shut doors when interviews, exams, procedures are being performed
• Staff ensure that only “authorized” persons review the individual’s record
• Individuals must give us permission to release information about themselves
• Avoid discussing the individual’s information in public areas
Issues or concerns regarding the care of a patient.
